What do I need for the registration process?
You must gather the following documents before beginning the registration application. This ensures that school staff can develop your student’s individual learning profile and supports for a smooth transition into their first day of school.

Parent/Guardian ID
- Valid U.S. driver’s license/ID card, passport, military ID, or other government-issued photo ID

One (1) Proof of Residency:
Temporary residence in Brookline (4 weeks or less), or residency for the sole purpose of attending public schools, will not be considered valid.
- Most recent mortgage payment
- Most recent property tax bill
- HUD-1 Settlement + payment
- Active leasing agreement
- Residency Certification Form (only if you are unable to provide proof in your own name)

Two (2) Proofs of Occupancy:
Utility statements, bills, or work orders dated within 45 days of your registration appointment. Must show service at your address in the parent/guardian’s name.
- Cable/Internet bill, statement of service, or work order
- Electric bill, statement of service, or work order
- Gas bill, statement of service, or work order
- Oil bill, statement of service, or work order
- Home telephone bill, statement of service, or work order
- Homeowner’s or renter’s insurance policy
- Proof of Occupancy Appeal Form and approved alternative documents (if applicable)

Documents About Your Student(s):
Must include authorized translations if not in English.
- Birth certificate or birth record
- Immunization records and most recent physical examination
- Progress reports or transcripts (based on grade level)
Required records by grade level:
- Grades 1–8: Most recent report card or progress report
- Grade 9: Grade 7 and 8 transcripts
- Grade 10: Grade 8 and 9 transcripts
- Grade 11: Grade 9 and 10 transcripts
- Grade 12: Grades 9–11 transcripts

Other Important Documents (if applicable):
- Active custody documents
- Active court records
- IEPs, 504 plans, or early intervention services
Please note that these documents are required to complete the online registration. Original copies must also be presented during your in-person appointment with the Office of Registration and Enrollment.
Failure to provide these documents at your appointment may delay enrollment. Students cannot begin school until all required documentation has been received.

For parents/guardians/caregivers/students unable to provide proof of residency documentation in their own name. This form may be required if your situation includes:
- Tenant-at-will (month-to-month)
- Property held in a trust
- Living with family or friends
- Student age 18+ living with family or friends
- Sublet or sublease agreement
- Rental of a room in a Brookline residence
- Live-in employment (e.g., nanny, caretaker)
For parents/guardians/caregivers/students unable to provide proof of occupancy documentation in their own name. This form may be required if your situation includes:
- Utilities not paid directly by tenant (must be stated in lease)
- BHA residency agreements
- Shared utilities with roommates
- Living with family or friends
- Student age 18+ living with family or friends
- Sublet or sublease agreement
- Rental of a room in a Brookline residence
- Live-in employment (e.g., nanny, caretaker)
