What documents do I need to change or verify my student's address?
You must gather the following documents before beginning the residency verification or change of address application. To access a printable version of the Required Document Checklist (PDF), please review the file linked here.
- One (1) Proof of Residency: Temporary residence in Brookline (4 weeks or less), or residency established solely for the purpose of attending public schools, will not be accepted. This document must show intent for you and your student(s) to reside at the address.
- Most recent mortgage payment
- Most recent property tax bill
- HUD-1 Settlement + Payment
- Active leasing agreement
- Purchase and sale agreement
- One (1) Proof of Occupancy: Utility statements, bills, or work orders dated within 45 days of submission. Documents must show service established at the address in the parent/guardian/caregiver’s name.
- Cable/Internet bill, statement of service, or work order
- Electric bill, statement of service, or work order
- Gas bill, statement of service, or work order
- Oil bill, statement of service, or work order
- Home telephone bill, statement of service, or work order
- Homeowner’s or renter’s insurance policy
Failure to provide these documents may delay the address change or residency verification process. If you are unable to provide any of these documents, please contact the Office of Registration and Enrollment at enroll@psbma.org or (617) 264-6492 to discuss acceptable alternatives.
